THEON is a proposed healthcare foundation-model family and research program. A primary-care episode supplies history, symptoms, observations, tasks and missingness only under purpose-specific research permission and source lineage. The capability map groups six research obligations: longitudinal and multimodal understanding; medical knowledge, uncertainty, causal and policy reasoning; and coordination, robotics and retained research-procedure improvement. It describes intended capabilities rather than selected neural-network internals. The subordinate authored FHIR example preserves an explicit DiagnosticReport R1.result reference to Observation O1 while treating the S1-to-R1 service relation as a proposed link requiring review or abstention. The two first studies change different objects: TH-FM-01 trains a standard low-rank adapter while retaining its frozen base; TH-RP-01 holds the model fixed while changing an eligible research procedure. Both require independent assessment and separately authorized release. No trained THEON capability, measured improvement or clinical benefit is shown; sovereign operation and task-specific validation remain separate obligations.
